This application form is to be used for Canadian program certification of Production Packages. A ”production package” is defined as two or more co-productions or co-ventures undertaken by a Canadian production company, together with one or more non-Canadian production companies, where a production with minor foreign involvement that qualifies as a Canadian production is matched with a foreign production with minor Canadian involvement.

Filing the application

File electronically via My CRTC Account by attaching the application to the Cover Page. My CRTC Account allows you to securely submit documents to the Commission with a user ID and password.

Note that completion of Section 5.0 DECLARATION OF THE APPLICANT OR ITS DESIGNATED REPRESENTATIVE is mandatory, even though a signature is not required when filing the application using My CRTC Account. Applicants who file their application in this matter are not required to submit a hard copy of the application and its related documents.

When filing your application, please ensure that the file name of the documents attached to your application form includes a brief description of the document content. For example, "Doc1-Application"; "Doc2-Appendix 3, Mr. Smith's affidavit", and so forth.

This enables you to complete the downloaded application by inserting your response in bold letters immediately following the question. You may add lines to the tables if necessary but do not alter or delete any text from the application.

Every line of this application must be completed and all applicable supporting documents and appendices identified in the checklist (Supporting Documents – Line 6.0) must be attached. If a line does not apply, insert N/A (Not Applicable) in the space provided. All supporting documents must be signed and dated.

The Commission reserves the right to refuse the application form if it has not been duly completed, was filled by hand or has been altered. The onus will be on applicants to submit a clear application that provides all of the relevant information and supporting documentation.
