On 28 October 1995, the Commission received an application from Global Voice Corporation (Global), for exemption from contribution for access circuits.

1. In Telecom Order CRTC 96-759 dated 16 July 1996, the Commission approved Global's application effective 28 November 1995 subject to the receipt of a new affidavit signed and properly sworn.
|
2. The Commission also approved the application for exemption from contribution for administrative circuits effective the date of installation subject to the receipt of a new affidavit signed and properly sworn.

3. Bell Canada (Bell) advised the Commission on 26 November 1996 that Global still had not filed the proper affidavits with it.
|
4. Commission staff unsuccessfully attempted to contact Global on 28 November, 4 and 20 December 1996 and 8 January 1997.
|
5. By fax on 7 March 1997, Bell stated that Global had advised Bell on 6 January 1997, that it had dissolved its business and at Global's request all services were then discontinued.
|
6. Bell stated that it unsuccessfully attempted to locate the president of Global.
|
7. In light of the foregoing, the Commission orders that:

8. Given that Global has not complied with the Commission's directions to file the proper affidavits, Global's application is denied.
