WHEREAS Global included two unsigned affidavits dated 30 October 1995, that stated that Global (a service provider of Prepaid Calling Cards) uses Regular Business Lines and Centrex System Lines to connect to the public switched telephone network (PSTN);
WHEREAS both affidavits stated that these connections are classified as interconnecting circuits and are used solely by staff for administrative purposes;
WHEREAS in a third sworn (but not signed) affidavit dated 28 November 1995, Global stated that it subscribes to a Centrex System using four PSTN connections in addition to those described in its 30 October 1995 affidavit;
WHEREAS, in addition, Global attested that no interexchange private line services or local private line services, whether provided by Bell Canada or any other supplier, are connected to the referenced telephone switching equipment;
WHEREAS by fax dated 5 December 1995, Global filed three signed affidavits to replace the affidavits mentioned above;
WHEREAS, by letter dated 21 December 1995, Bell submitted that the revised affidavits meet the Commission's evidentiary requirements and, accordingly, agreed with the proposed exemptions;
WHEREAS the Commission notes that Global subscribers dial into Global's Centrex system through an 800 number and following which Global's equipment processes the call and routes it via dedicated lines to a long distance provider; and
WHEREAS the Commission notes that in its fax of 5 December 1995, it appears that Global has merely signed its previous affidavits without having them resworn -
IT IS HEREBY ORDERED THAT:
1. Global's application for exemption from contribution for access circuits is approved effective the date of application (28 November 1995), subject to the receipt of a new affidavit which is signed and properly sworn. The Commission notes that an affidavit must be signed and sworn at the same time in order to be valid.
2. Global's application for exemption from contribution on its administrative circuits is approved effective the date of installation, subject to receipt of properly signed and sworn affidavits.
