Subject: Information to be provided in customer notifications associated with tariff applications to destandardize and/or withdraw services
In Telecom Information Bulletin 2010-455-1, among other things, the Commission sets out the filling information that is required to be provided for applications to destandardize and/or withdraw tariffed services.
One of the requirements is that the applicant must provide notice to each customer affected by its application to destandardize and/or withdraw a particular service. In the notice, the applicant must include the following information:
- a description of the service proposed to be destandardized and/or withdrawn;
- the proposed date for the destandardization and/or withdrawal;
- the rationale for the destandardization and/or withdrawal;
- the type of destandardization, where applicable; and
- clear and detailed information about how an affected customer can participate in the Commission’s process, including the date by which interventions must be received by the Commission.
Where customers provide comments through the comment button on the Commission’s website related to a specific application, a privacy statement will be displayed. However, if a customer submits comments by postal mail, fax, or email, the customer may not be aware that the information that they are submitting will become part of the Commission’s public record.
Accordingly, Commission staff requests that applicants include the following information in their notifications to better inform customers when preparing their comments:
All information, whether sent by postal mail, fax, email, or through the Commission’s website at www.crtc.gc.ca, becomes part of a publicly accessible file and will be posted on the Commission’s website. This includes all personal information, such as full names, email addresses, postal/street addresses, and telephone and fax numbers.
The personal information that parties provide will be used and may be disclosed for the purpose for which the information was obtained or compiled by the Commission, or for a use consistent with that purpose.
