1. With regard to funding used to support the current 9 1 1 system, complete the following table identifying revenue that the company derives from the various sources, specifying specific total dollar amounts for year 2012.

2. Provide a list and map of exchanges in your operating territory, identifying those exchanges where your customers can dial to 9-1-1 to reach emergency services, and those exchanges where they cannot. Also, for those exchanges where your customers can dial 9-1-1, identify:
a. whether Enhanced 9-1-1 is provided, or whether only basic 9-1-1 is provided;
b. whether your company self-supplies 9-1-1, or whether it uses a third-party carrier’s service and, if so, whose; and
c. the % of lines within each exchange for which dialing to 9-1-1 is provided.

The company is requested to submit responses using both My CRTC Account and the Commission’s Data Collection System.
