ANNEX 1
1. Identify all of the information entered into the E9-1-1 database for the Companies’ multi-line customers. In particular, indicate whether the Companies include all or some of the following information and, if so, in what circumstances the information is included:
(i) Working Telephone Numbers (WTNs)[1] associated with an outgoing access line or DS-0 equipped for outward calling;
(ii) WTNs associated with DIDs that are not equipped for outward calling;
(iii) WTNs associated with Direct Inward Dial lines (DIDs) that are also equipped for outward calling;
(iv) extension number information for extensions that are not DIDs; and
(v) location information associated with any of the above categories (e.g., the building address for outgoing access lines or an office number associated with a DID or an extension)
2. Identify all of the information that appears to the PSAP operator when an end-user of a multi-line customer dials E9-1-1. In particular, indicate whether the information identified in the Companies’ response to question 1 appears to the PSAP operator in all cases.
3. For the categories of information identified in question 1 that are not included in the E9-1-1 database:
(i) Indicate whether it would be possible for the Companies to obtain this information for all of their multi-line customers.
(ii) Identify any additional costs that would be incurred to obtain this information with supporting rationale.
(iii) Indicate whether including some or all of this information in the E9-1-1 database could improve public safety.
Identify, explain and provide some examples of under what circumstances, for multi-line customers, the company creates records in the 9-1-1 database based on the customer’s WTN or based on NAS. Please indicate whether subsequently it is possible to distinguish which 9-1-1 database records were created based on the customer’s WTN or based on NAS.
Annex 2
