Dear M./Ms,

RE: Activation code for CRTC 2005 Data Collection System (DCS)

This letter is issued to all reporting entity users of the on-line Data Collection System (DCS). As per Telecom Circular CRTC 2005-4 , 9 February 2005 , paragraphs 5 & 6, the CRTC is using the Government of Canada's epass system to communicate securely with on-line Government services.

The 2005 Telecommunications Industry Data Collection will commence on February 18, 2005 . All users are required to use a new or existing epass with the following one-time activation code for access to DCS.

Activation Code: xxxxxxxx

Please note that this activation code will expire after 21 days.

To activate your epass and log into DCS, visit: http://www.crtc.gc.ca/eng/dcs/login.htm. From here you can also learn more about epass, read information about how to obtain an epass, or review the frequently asked questions about epass.

If you have any questions about DCS or epass, or have problems with the system, we can be reached by e-mail at telinfo@crtc.gc.ca, by phone at (819) 997-4597, or by fax at (819) 953-0795.

Sincerely,

Stephen Delaney
