Legal name of reporting entity: Re: Monitoring the Canadian Telecommunications Industry - 2003 Data Collection On 2 April 2003, a letter was issued to [reporting entity], identifying which Data Forms were to be submitted to the CRTC by 15 May 2003. The attachment to the 2 April 2003 letter contained errors in the listing of your Data Form requirements. We are forwarding today a revised attachment that lists the specific Data Forms that apply to your organization, with any revisions identified by means of an R prefixed to the form number. The Data Forms to be completed by 15 May 2003 should therefore include all of the forms set out in the attached 2003 Survey Data Form Requirements (Revised). Revisions have also been made to some of the forms that are downloadable from the Canadian Telecommunications Markets: Forms page of the CRTC's Web site on 2 April 2003. Forms 03-310 Income Statement and 03-315 Operating Revenues-Business (Small, Medium, Large & Very Large) have been updated to correct some column headings and cell references within the notes and input areas. These changes are highlighted in bold. In addition, problems related to cells that were inadvertently locked in Forms 03-4XX Data & Private Line, Forms 03-5XX Internet, Form 03-611 Local & Access - NAS and 03-8XX Mobile & Paging have now also been corrected. Revised forms are now available for download from the CRTC Web site. Submissions due 15 May 2003 are to be made using the revised forms. We apologize for any inconvenience that this may cause.
