IN THE MATTER OF an application filed by Ad-Tel Communications, on behalf of Peoples Telecom Corp. (Peoples Telecom) on 3 January 1997, requesting an exemption from contribution charges for Centrex services used to provide single-hop service, and for administrative lines.

WHEREAS in support of its application, Peoples Telecom provided an affidavit dated 7 January 1997 affirming that the services are used solely for the applicant's administrative use or to provide single-hop services;
WHEREAS by letter dated 6 February 1997, Bell Canada (Bell) noted that the services in question include a Centrex system and a business line which are due to be installed shortly;
WHEREAS in this respect, Bell also noted that a sworn affidavit is considered to be satisfactory evidence in support of applications for exemptions for both administrative and single-hop exemptions;
WHEREAS however, Bell submitted that the affidavit provided is unsatisfactory in that it predates the installation date of the services and makes no reference to the business line used for administrative purposes;
WHEREAS based on the evidence provided, therefore, Bell agreed with the requested exemption, subject to the receipt of a revised affidavit including references to all services which are the subject of Peoples Telecom's application; and
WHEREAS the Commission agrees with Bell's view that an affidavit meets the Commission's evidentiary requirements but that the affidavit provided is unsatisfactory -
IT IS HEREBY ORDERED THAT:
Peoples Telecom's application is approved effective the date of installation, subject to the receipt of a revised affidavit reflecting the actual services within 30 days of their installation.
