IN THE MATTER OF an application filed by the Ontario Telephone Association under Tariff Notice 21 dated 3 October 1996, as amended by Tariff Notices 21A and 21B dated 20 December 1996, for approval of tariff revisions providing modifications to the Terms of Service, filed further to Telecom Decision CRTC 96-6.
