By letters dated 27 May and 1 June 1998 (enclosing an affidavit dated 3 June 1998), Fone Pass requested an exemption from contribution charges with respect to Centrex systems used solely to provide local or single-hop services.

1.By letter dated 30 June 1998, Bell Canada (Bell) noted that the affidavit is deficient in one respect. Bell stated that Fone Pass has provided neither a list of the specific Centrex systems that require exemption nor the billing telephone number or the Numbering Plan Area (NPA) NXX associated with the systems.
2.Bell noted that in the covering letter dated 27 May 1998, Fone Pass indicated that an "order for facilities will follow approval". In light of this, Bell assumed that the Centrex services have not been installed and, therefore, recommended that the Fone Pass application be characterized as a request for advance approval for an exemption. In this respect, Bell stated that the application appears to satisfy the evidentiary requirement for such exemptions.
3.Accordingly, Bell agreed with the request for advance approval of an exemption. Should the Commission grant Fone Pass' request, Bell also submitted that Fone Pass should be required to provide a revised affidavit after the installation of its first system, identifying the system and affirming that it is arranged in a manner to limit Fone Pass' services to local or single-hop configurations.
4.The Commission is of the view that Fone Pass has provided an affidavit which meets the evidentiary requirements for advance approval of single-hop services and notes that Bell also agrees.
5.In light of the foregoing, Fone Pass' application is approved effective the date of installation, subject to Fone Pass providing a revised affidavit within 30 days after the installation of its first system, identifying the system, and affirming that it is arranged in a manner to limit Fone Pass' services to local or single-hop configurations.
